Proposed Order to Employer to Pay the Trustee

Background:
Effective June 12, 2015, an attorney or Trustee may e-file a Proposed Order to Employer to Pay the Trustee for Chapter 13 cases in the Eastern Division.
Local Form, #25: Order to Employer to Pay the Trustee
The court created a new fillable form, Local Form, #25, Order to Employer to Pay the Trustee. The form will allow the filer to select the Judge and Trustee and enter the name of the employer, debtor and deduction amount in the body of the order. The form requires attorneys to sign with a /s/ and date the appropriate fields. Debtors must sign with a handwritten signature, and the document should be scanned. Please check the appropriate checkbox found at the bottom of the form.

This form MUST be filed using the new event "Proposed Order to Employer to Pay the Trustee." After review by the Clerk's office staff, the order will be dated, signed, and entered on the docket.
Step-by-Step Procedures:

Select [Bankruptcy>Trustee/US Trustee]

Enter case number and click Next

Select Proposed Order to Employer to Pay the Trustee from event pick list and click Next

Select Trustee from the party pick list and click Next

The PDF Document selection screen will display.

Click Next

The Modify Text Screen will display.

If applicable, click the down arrow to choose a prefix from the pick list

Verify the same prefix is included in the title of the PDF

Reminder : Only add Amended to the PDF and the docket text if a previous order was entered.

Click Next

The Final Text screen will display. This is your final opportunity to modify the entry before submitting the filing.

The Notice of Electronic Filing will display. It is the verification that the filing has been sent electronically to the court.
